We help elders – regardless of age or altered abilities – achieve the most independent living environment possible and enjoy a high quality of life in a safe, secure and pleasant setting.

We’re all aging, every day, and many of us are able to live decades – sometimes our entire lives – with minimal assistance from others. As you face the challenges of growing older, HV Aging at Home can help you and your family assess what’s needed to give you the highest quality of life while aging at home or in other settings.

Though our primary focus is on helping people age comfortably and safely at home – usually the least restrictive and familiar environment – we also work with you and your family to ensure proper care and support is given if you live in a nursing home or an assisted living facility. While having a medical, mental or physical disability may be an impediment, you still have options. As your advocate, we’ll help you to evaluate and overcome obstacles wherever you are living – at home, in a skilled nursing or assisted living facility, in hospital or hospice.

Working alongside your network of family, friends and caregivers, we’ll help you get the essential medical care, home modifications, recreational activities and other services you need to enjoy life more fully.

We provide oversight of elder/disability plans; help you find (or receive benefits to pay for) contracted services; assist in evaluating your needs for legal and financial planning; offer advocacy to obtain maximum services and supports; coordinate with Managed Care, facilities and insurance agencies; and offer other assistance, as needed.

On a personal side, we’ll work with you to maintain your ties to family and community, and help you enjoy your creative, recreational, professional and intellectual interests, too. Most people have significant concerns about managing both the costs and details of living at home versus choosing an assisted living facility or nursing home. To fully evaluate your unique situation, it’s often necessary to coordinate planning and implementation of numerous agencies and financial tools to guide medical decisions that can be difficult for you, your proxy and concerned family members.

The bottom line is that making these arrangements, especially in advance, offers protection not only for you, but for your family members and anyone charged with the responsibilities of Trustee, Power of Attorney, Health Care Agent or Guardian.
